*Emergency card can not be downloaded. It must be picked up in the Athletic Training Room.
DO NOT TURN PAPERWORK IN TO A COACH OR MAIN OFFICE!
Dr. form must be filled out and on file with the athletic trainers for any injury, illness, or dental visit
Athletic Training Room Hours:
Mon, Wed, Thur 7:30am - 8:45 am Be here NO LATER than 8:15am
Tuesday and Friday 8:00am-8:45 am Be here NO LATER than 8:15
Monday-Thursday 4:10pm - 5:15pm Be here NO LATER than 4:45pm
Treatments are also available the last 30 minutes of the athletic block to those who come in for morning treatments
